Classification and Treatment of Posttransplant Lymphoproliferative Disorders
نویسنده
چکیده
ABSTR ACT: Posttransplant lymphoproliferative disorders (PTLDs) are defined by the 2008 World Health Organization classification. Monomorphic PTLD is the most frequent form; it is usually diagnosed several years after transplantation and nowadays is positive for Epstein–Barr virus in only 50% of cases. Although preventive treatments are not effective, in cases of Epstein–Barr virus reactivation, a preemptive approach can prevent the development of PTLD. The first-line curative treatment consists of reducing immunosuppression, where possible, and this alone can cure PTLD. If this fails, rituximab monotherapy is safe and induces complete remission in one-third of cases. If complete remission is not achieved, four cycles of R-CHOP [cyclophosphamide, hydroxy doxorubicin, vincristine (Oncovin®), and prednisone plus rituximab] are generally sufficient.

OBJECTIVES Hepatitis C virus infection has a 10.5% frequency in liver transplant posttransplant lymphoproliferative disorders. Studies have suggested that hepatitis C virus infection plays a role in developing posttransplant lymphoproliferative disorders. Pooling data of posttransplant lymphoproliferative disorders developing in liver recipients from the literature, we analyzed and compared cha...

Posttransplant lymphoproliferative disorder (PTLD) is a rare but life threatening disorder following both solid organ and hematopoietic stem cell transplantation. The disorder is characterized by an uncontrolled proliferation of lymphocytes, caused by medication induced diminished immune surveillance. From a pathological point of view PTLD has a broad and heterogeneous spectrum of appearance, r...

The Epstein-Barr virus (EBV) has a pivotal pathophysiologic role in the development of most lymphoproliferative disorders that occur after solid-organ transplantation. The term "EBV-associated posttransplant lymphoproliferative disorder" (PTLD) includes all clinical syndromes of EBV-associated lymphoproliferation, ranging from uncomplicated posttransplant infectious mononucleosis to true malign...
